Community-Based Support for Independent Living

At Smith’s Graceland Care, LLC, we provide non-medical, person-centered services that support seniors and individuals with developmental disabilities in living safely, independently, and connected to their communities. Services are delivered in the home, with family, independently, or in supported residential settings, based on individual needs and approved care plans.

Our focus is always on dignity, choice, independence, and community inclusion.


Community Integration and Habilitation

These services support individuals in building daily living skills, maintaining independence, and participating meaningfully in their communities.

Residential Habilitation and Supports

Assistance provided in a home or shared living environment to support:

  • Daily living skills and personal routines
  • Household responsibilities and independent living skills
  • Community participation and social engagement
  • Personal choice, dignity, and self-direction

Remote Supports

Technology-assisted services designed to promote safety and independence when staff are not physically present, including:

  • Ongoing supervision and wellness monitoring
  • Support aligned with approved care plans
  • Encouragement of independence while maintaining safety

Wellness Coordination

Non-medical support focused on overall well-being, including:

  • Assistance with scheduling and attending appointments
  • Care coordination and communication support
  • Health-related reminders and routine management
  • Promoting consistent, healthy daily routines

Family Support Waiver (FSW) Services

Personal Assistance Care (PAC)

Through the Family Support Waiver, we provide one-on-one support to individuals living with their families or independently. Services are tailored to individual goals and may include:

  • Support with daily activities and personal routines
  • Skill-building to increase independence
  • Assistance accessing the community
  • Support in achieving personal, social, and household goals

Money Follows the Person (MFP)

We support individuals transitioning from institutional settings back into the community and maintaining successful independent living.

Under the MFP program, we offer:

  • Residential Habilitation and Supports
  • Remote Supports
  • Wellness Coordination

These services are designed to support a smooth transition, promote independence, and help individuals remain successfully integrated into their communities.


Personal Care Services Agency (PSA)

Through our PSA services, we primarily provide attendant care and daily living support for seniors and eligible individuals.

Attendant Care & Daily Living Assistance

Services may include:

  • Assistance with activities of daily living (ADLs), such as bathing, dressing, grooming, and mobility
  • Meal preparation and light household tasks
  • Errands, grocery shopping, and transportation assistance
  • Community support to maintain daily functioning and independence
